This is not a standard dental intake or symptom checklist — it's a physical therapy evaluation of how your jaw, neck, and posture are functioning together.
This consultation is designed for individuals who are looking for a more focused, one-on-one evaluation of persistent or unresolved TMJ symptoms—especially after other approaches have not provided clear answers.
You will be working directly with Chris Junker, PT, from the start.
This is a private, self-pay service with limited availability, and each request is reviewed to determine whether this approach is the right fit.

It is a focused, clinical conversation and evaluation process designed to:
Understand the full history of your symptoms
Review what you’ve already tried
Identify patterns that may have been overlooked
Determine whether a more detailed TMJ-focused approach is appropriate
Determine whether a physical therapy–based, non-dental approach may address what previous treatment has not
For many patients, this is the first time their situation is examined in a comprehensive and integrated way.

An insurance-based consultation
A one-time visit focused only on short-term relief
A dental or oral-appliance-based evaluation

How This Process Works
The consultation process
Step 1 — Submit a Request
You begin by completing a short consultation request form.
This includes:
Your symptoms
Duration of the issue
Previous treatments (dental, PT, medication, etc.)
What you are currently looking for
Each submission is personally reviewed.
Step 2 — Initial Review & Contact
If your situation appears to be a potential fit, you will be contacted directly.
At that point, I may:
Clarify your symptoms
Discuss your previous experiences
Determine whether a full evaluation is appropriate
This step ensures that time is used effectively for both sides.
Step 3 — In-Person Evaluation (If Appropriate)
If we're a good fit, I'll schedule you for a comprehensive, one-on-one evaluation.
This is where I begin a deeper assessment of:
Jaw mechanics
Muscle involvement
Neck and postural contribution
Behavioral and movement patterns
This evaluation is the foundation for everything that follows.

One-on-one sessions with Chris Junker, PT
Individually tailored based on your presentation
Adjusted in real time based on your response
Care may involve:
Manual therapy and intraoral work (when appropriate)
Movement retraining and motor control work
Postural and cervical contributions
Habit and behavior modification
Progressive exercise and coordination training

No assistants

No high-volume scheduling

A Self-Pay, Private Care Model
This practice operates outside of insurance.

Self-pay practice — 30+ years of physical therapy experience, with a clinical focus exclusively on TMJ and orofacial pain for the past 7+ years.
Initial Evaluation — 45 minutes — $205
A comprehensive assessment of your jaw, posture, and history, resulting in a personalized plan of care.
Follow-Up Session — 30 minutes — $180
Focused, hands-on treatment as part of your ongoing plan.
A detailed receipt (Superbill) is available upon request for out-of-network insurance reimbursement.
Before moving forward with any treatment, I will clearly review:
The findings from your evaluation
A recommended plan of care
The expected structure and frequency of visits
All associated costs
This allows you to make a fully informed decision before committing to care.
